How to join our program:
You or your loved one may become a member if:

- Minimum requirements to become a member:
- At least 16 years old
- You live with or are willing to live with a qualified caretaker in a home-based setting
- Qualified home setting
- Comply with MassHealth and CARF standards
- Eligibility is determined through a comprehensive nursing and social work assessment.
Minimum requirements to become a caretaker:
- Caretakers can be family members (not a spouse or legal guardian) or non-relatives committed to providing consistent care in their homes.
- At least 18 years old
- Pass background checks (CORI, fingerprinting)
- Qualified home setting
- Comply with MassHealth and CARF standards

All members have the right to:
- Be treated with dignity and respect
- Receive care in a culturally appropriate and person-centered manner
- Participate in developing their care plan
- Refuse services
- Confidentiality of health records
- Report concerns without fear of retaliation
Our staff is trained on HIPAA, privacy, abuse prevention, and CARF’s Rights of Persons Served.
